The CPT divides services into six categories, each of which represents a chapter in the book:

Evaluation and Management (E/M)

 Anesthesia

Surgery

Radiology

Pathology and Laboratory

Medicine. 

Codes are listed numerically within the CPT with one exception: the E/M chapter is listed first because the procedures and services in it are so frequently used.

Each of these sections is divided into subsections, subheadings, categories, and subcategories, all of which are further divisions of sections that are based on anatomy, procedure, condition, description, or approach.
